Get started on the path to zero patient harm!
To reduce harm from pressure injuries in surgical patients through risk assessment and strategic planning in the perioperative setting.
For over 100 years perioperative pressure injuries (PPI) have occurred in surgical patients, and in many cases assumed to be unavoidable. The primary cause of pressure injury is the sustained exposure to cell and tissue deformation. This is often amplified by the challenges in the operating room (OR), prolonged immobility, extreme positions, and utilization of rigid medical devices.
The Scott Triggers® is the first evidence-based pressure injury risk assessment tool designed specifically for surgical patients. Significant reduction in pressure injuries has been reported by utilizing the Scott Triggers® with an evidence-based skin bundle and strategic plan.
The Scott Triggers® tool is the first step to creating a lean standardized Quality Improvement Solution. When integrated into the electronic medical record it allows for real-time alerts to the perioperative staff of patient risk. By linking high-risk individuals with the right assessments and right interventions, improved outcomes are achieved.
